The support you receive from your Support Coordinator will depend on where you are on your journey, what support you need in your life and if the NDIA requires certain things to be done to support your journey. Our Support Coordinators will work with you to map out the support they can provide you within your funding, which may include the following:
- Exploring options: Assess all available supports, including mainstream, community, informal, and funded services.
- Connecting with providers: Assist in selecting providers, negotiating agreements, and managing service bookings.
- Organising assessments: Coordinate assessments to determine appropriate funding, such as functional capacity or home modifications.
- Managing budgets: Allocate funding across supports and ensure correct claims with your plan manager.
- Linking to community supports: Connect you with broader services like housing, health, education, and transport.
- Building your capacity: Empower you to understand your rights, resolve issues, and gain skills to manage and direct your own supports.
- Supporting life transitions: Guide you through key life stages, plan reviews, new goals, and long-term housing options.
