How do I get connected with Community Participation?

Getting started with Community Participation support can be done in a few ways. You can request support through: 

  • The referral form below
  • By emailing through to [email protected]
  • Calling our reception on 02 4944 5900 and asking to speak with our NDIS Intake team.

Once we receive your referral, you'll get a confirmation email to let you know it's been received. From there, a member of our Community Participation team will be in touch to provide you with an intake form or complete an intake meeting in line with your preferences.
